Please fill these out within 24 hours of booking your appointment or your slot may be cancelled. If you don't see these forms in your inbox, please check spam as they sometimes end up there. Every massage appointment includes an intake and home care. Direct Billing is available for most insurance plans. Please let me know if you would like to set this up and I will email you the forms to fill out prior to your appointment. Otherwise, payment is due in full, immediately after your appointment. Bowen Therapy + Rapid NFR Treatments: Please wear loose, comfortable clothing. Treatments are done without lotion and over your clothing.
